Health-Related Social Needs (HRSN) Screening Tool

The Health-Related Social Needs (HRSN) Screening Tool was developed by the Centers for Medicare & Medicaid Services Center for Medicare and Medicaid Innovation to analyze if the health-related social needs of Medicare or Medicaid beneficiaries are being met, as well as if this has any effect on health care costs and health outcomes. The tool was developed by a series of experts in various fields, including public health researchers, clinicians, federal partners, and other professionals.

Estimation Technique

This tool can be used by healthcare providers to assess the social needs and risk factors of patients. The tool groups patients’ needs into five domains, one of which is transportation problems that impact their health. It includes questions regarding transportation to essential destinations and opportunities to engage in physical activity. It also includes questions related to physical and mental conditions that prevent the patient from doing errands alone, mental health assessments, and how often the patient feels lonely or isolated from those around them.
